How AgeWell Care Works — From First Call to Long-Term

Most people come to AgeWell because something has changed. Their energy is different. Their sleep is different. Their drive, weight, mood, strength, libido, confidence, or connection may not feel the way it used to. Many have been told their labs are “normal,” but they know something is off.

The AgeWell Pathway is our step-by-step process for helping you understand what is changing in your body, take action with confidence, and age on your own terms.

 

Step 1

The Discovery Call

It starts with a conversation, not a commitment.

You'll talk with our team about what's going on, what you've already tried, what you want to feel or function differently, and whether AgeWell is likely the right fit. We'll explain how AgeWell works, what the first step includes, what it costs, and what the broader care pathway may look like. If you share health or financial decisions with a spouse or partner, we encourage you to include them on the call.

Step 2

The Foundational Assessment

The Foundational Assessment is the first step for every new AgeWell patient. This is where we stop guessing and start connecting the dots.

Your assessment includes a comprehensive review of your symptoms, story, goals, labs, health history, and current concerns, so your provider can understand what may be changing and what kind of support makes sense. We look at the whole picture: hormones, metabolism, thyroid, inflammation, cardiovascular markers, lifestyle, sexual health, symptoms, goals, and the life behind the labs.

AgeWell welcomes individuals and couples. If you choose to move through the process with a spouse or partner, each person receives their own assessment and labs, with visits coordinated when appropriate.

The Foundational Assessment is an evaluation and recommendation visit. It is designed to help your provider understand what is changing and recommend an appropriate care pathway. Prescriptions and medication management begin when you choose to proceed into an AgeWell clinical program, when clinically appropriate.

Step 3

Structured Program Care

Your provider will recommend a clinical program designed around the key drivers of how you feel and function.

This is the next step for patients who are interested in treatment, prescriptions when clinically appropriate, and ongoing personalized care. Your program may focus on hormones, metabolism, sexual health, nutrition, lifestyle, recovery, mindset, or a combination of these areas.

Care begins with a Treatment Plan Initiation visit, where your plan becomes specific:

  • what we are addressing
  • why it matters
  • what tools we are using, including advanced lab testing, prescriptions for hormones or peptides, and lifestyle modifications
  • how we will monitor your response

Programs typically run for three to six months and include follow-up visits, lab tests, supplements, prescription management, lifestyle guidance, and ongoing support from your care team.

Step 4

Membership-Based Ongoing Care

Feeling better is where we start, not where we stop.

After completing a program, many patients transition into membership for ongoing care, prescription management, monitoring, and long-term optimization. Membership allows us to continue tracking labs, symptoms, medication response, prescriptions, goals, and prevention over time. It is designed for patients who want continued support as their body, life, and health priorities evolve.
